Registration

REGISTRATIONS FOR ALL INCOMING STUDENTS WILL START ON JUNE 24th, 8:00 AM – NOON.

  • INCOMING 9TH GRADERS: UNDER ITEM NUMBER 5 – SCHOOL RECORDS, THIS IS THE FINAL 8th GRADE REPORT CARD.
  • ALL OTHER GRADE LEVELS:  UNDER ITEM NUMBER 5 – SCHOOL RECORDS MUST BE FOR THE LAST 3 YEARS.

PARENTS REGISTERING THEIR CHILD MUST COMPLETE STEPS 1-6 LISTED BELOW.

NEW Students to Miami-Dade County Public Schools MUST present the following documentation to REGISTER student at Alonzo & Tracy Mourning Senior. 

1.  Proof of address – Please provide (2) of the following documents listed below NO OTHER DOCUMENTS CAN BE ACCEPTED

  • Warranty Deed or Notarized Lease Agreement 
  • Most current FPL Bill

2.  Student’s ORIGINAL Birth Certificate & parent identification.

3. Immunization Records (converted to Florida form #680 blue form –    full page version is the only form allowed)

4. Physical Exam Record (Including Tuberculosis Test results) #3040 yellow form

5. School Records from previous school (documents other than English, Spanish, or Creole must be translated to English)

6.   Please print all documents and bring completed items to the school with you.

WE ARE NOT A MAGNET SCHOOL THEREFORE THERE IS NO APPLICATION PROCESS!


TRANSFER Students from another Miami-Dade County Public School

MUST present the following documents to REGISTER:

1.    Proof of address – Please provide (2) of the following documents below NO OTHER DOCUMENTS CAN BE ACCEPTED: 

  •     Warranty Deed or Notarized Lease Agreement and 
  •     Most current FPL Bill 

2.   Withdrawal papers from the previous Miami-Dade County Public School

3.  Parent Picture Identification with child’s birth certificate.

4.    10 Page Registration Application in LINKS section.

WITHDRAWING Students to another MDCPS school or other school MUST present the following documents to REGISTER:

1.   Name and Address of new school that student will be attending.

2.   Parent Picture Identification

3.    Parent who registered student MUST be present at the time of Withdrawal
